Looks like you're missing the firmware. I think what you need is to download the following:
http://dl.ivtvdriver.org/ivtv/firmware/cx18-firmware.tar.gz
Untar and put the .fw files in /lib/firmware. FMI, see:

FWIW, I use the mythbuntu variant of ubuntu and these files were automatically installed for me. On mythbuntu/ubuntu, they come from the linux-firmware package.
